3.1 How can request for a certification? 

For certification that covers Honors, Awards, Certificate of Enrolment for Visa Application, Ranking, Graduation, English as a Medium of Instruction, Diploma Translation, Attendance and Candidacy for Graduation, please send a letter of request via This email address is being protected from spambots. The letter of request must indicate the purpose of the certification. Documents are available after 3-5 working days. 

 

3.2 How do I get Transcript of Records (TOR)? 

 A letter of request shall be forwarded through the official email addressThis email address is being protected from spambots. Attach the official request from the school where the concerned student is enrolled. The Registrar’s Office sends the credential to the receiving school through courier (D.O. No. 54, s.2016).

 

3.3  When is a Transcript of Records (TOR) requested? What purposes are usually considered?

Transcript of Records is requested for Studies Abroad, Further Studies, and Employment Purposes.  

Processing of said documents will take 7 to 10 working days. Payment can be made onsite or online.

3.4 How do we request for Certificate of Good Moral Character and Recommendation Form/Letter from the Guidance Office or the Student Development Center? 

Certificate of Good Moral Character and Recommendation Form/Letter may be requested from the Student Development Center by sending an email to This email address is being protected from spambots. You need JavaScript enabled to view it.. Indicate the purpose for request, your name, grade and section, and the e-mail address of the school where the concerned student is applying for admission. 

 

3.5 How do we apply for ESC/SHS Voucher Program Grants? 

CSQC is a participating school in the Education Service Contracting (ESC) and Senior High School (SHS) Voucher Programs of the Department of Education through the Private Education Assistance Committee (PEAC). 

 

For the ESC Grant: 

Grade 7 students may apply for the ESC Grant. Learners and their parents/guardians are responsible for their own applications. For more details on eligibility and procedure, please check DepEd Order No.20, s.2017. Updates are posted in the Registrar Channel of the Student and Parent Central(Microsoft Teams), and CSQC’s Official WebsiteandClaret Facebook page. 

 

For the SHS Voucher Program: 

Grade 11 students may apply for the SHS Voucher Program. Learners and their parents/guardians are responsible for their own voucher applications. Grade 10 completers in Private Schools who are ESC grantees are considered Automatic Qualified Learners and do not need to apply for the SHS Voucher program.
